Description

The U.S. Naval Hospital Guam intends to award to a responsible quoter for the service of one (1) Registered Dental Hygienist for the CNM Guam Branch Dental Clinic. Travel, housing or relocating expense to Guam will not be covered for this award. The North American Industry Classification System (NAICS) Code is 621399 (Size Standard: $6.5 million). All duties shall be performed in accordance with standards and methods generally accepted within the field of Dental Hygienist services and in compliance with all Federal and State policy and regulations. Contractor shall adhere to all standards and regulations associated with the Joint Commission on accreditation of Healthcare Organizations. Contractor's performance shall be monitored by the COTR. Contractor shall have at least one year postgraduate experience as a registered dental hygienist and a degree from an ADA accredited school of dental hygiene and must possess and maintain a current license and registration to practice the required service without restrictions. Contractor's name, resume and photocopies of documents evidencing education and training must be submitted with the quotation. The contract period will be for ten (10) months. Solicitation will be available on or about April 08, 2008. No phone calls accepted. The anticipated receipt of quotes is April 18, 2008. The solicitation is restricted to small business sources.
