Attachment C Past Performance Questionnaire Attachment B Sample Postcard and Letter Attachment A Surveillance Plan RFQ Electronic Print to Mail This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 052P1700804. The solicitation is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-95. The North American Industry Classification System (NAICS) code for this acquisition is ALL OTHER BUSINESS SUPPORT SERVICES (561499 ). The small business size standard in number of employees is $15,000,000.00. This solicitation is a total set-aside for small business. The Period of Performance for this requirement is a one year base period and four one year option period. This is a solicitation for printing and mailing of USPTO paper notices to stakeholders and customers. Currently, the majority of notices are sent as postcards. However, some notices are letters with a variety of multi-page and double-sided printing requirements. All letters and postcards shall be prepared according to the United States Postal Service (USPS) policies for the handling at the most cost-effective USPS rate available to achieve maximum cost savings. The vast majority of letters and postcards are for domestic mailing, however, there are some postcards and some letters with international addresses. It is anticipated that specific quantities requirements will change over the life of this contract. Printing and mailing of USPTO paper requirements have been known to fluctuate greatly over short periods of time. Therefore, the Government contemplates the award of an Indefinite Delivery Indefinite Quantity (IDIQ) requirements contract. All questions pertaining to the RFQ shall be submitted electronically to Hope Smith, Contracting Officer, at Hope.Smith@uspto.gov and Dru Olson at dru.olson@uspto.gov. Questions must identify the author and company name. All questions and responses pertaining to the RFQ will be published and made available on FBO.gov. All questions regarding the RFQ are due by 1:00 pm, Eastern Standard Time (EST), November 9, 2017. Offerors shall include in the subject line of e-mail "Questions - RFQ 052P1700804." The government will not accept questions submitted after the deadline. Quotes and supporting documentation are due from offerors by 5:00 pm, EST, November 20, 2017, and shall be submitted via e-mail to Hope Smith, Contracting Officer, at Hope.Smith@uspto.gov and Dru Olson at dru.olson@uspto.gov. Subject line on the e-mail shall be "Quote Submittal RFQ 052P1700804." The USPTO reserves the right to amend the terms of this RFQ at any time prior to the source selection decision. Offerors shall be responsible for accessing FBO.gov for any amendments to the solicitation. All amendments will be posted at this location.
