The Department of Veterans Affairs (VA), Hudson Valley Health Care System, VA Medical Center is currently conducting a market survey for qualified firms capable of providing all labor, supplies, equipment, maintenance, information technology, and supervision necessary to provide Protestant service to Hudson Valley Health Care System, Montrose VAMC, NY, described herein. Protestant Chaplain Services include, but are not limited to: PROTESTANT CHAPLAIN STATEMENT OF WORK PURPOSE: The purpose of this contract is for provision of Sunday Protestant Worship services at the VA Hudson Valley HCS, Montrose Campus Chapel to veteran patients of the VA Hudson Valley Healthcare Systems. DEFINITIONS: In this document the term Chaplain refers to individual providing services as described. SERVICES: Every Sunday at 10:00AM. The Service generally lasts one hour. The Chaplain is responsible for planning of the service; and set up and teardown after. CONTRACT PERIOD November 1, 2017 through September 30, 2018 Estimated Quantities: 52 All quantities listed herein are estimated annual quantities. CONTRACTOR QUALIFICATIONS 1. The chaplain must meet the qualification requirements for Chaplain GS-060-11 given in VA Handbook 5005 Part II, Appendix F1. (See attachment 2) The Director, Chaplain Service, or designee, at the VA National Chaplain Center in Hampton, Virginia, will verify and certify that the chaplain meets these requirements prior to any work being performed under this contract. 2. The Chaplain will provide VA with the following documents and any other information and/or documents requested by the Director, Chaplain Service, for each chaplain performing work under this contract, to be used to verify the qualifications of the chaplain. Documents required: 1) Ecclesiastical endorsement by the properly designated national endorsing agent of a religious faith group, dated within the past 12 months. The Director, Chaplain Service, at the VA National Chaplain Center will verify that the endorsement is proper before the chaplain performs any work under this contract. 2) A resume, curriculum vitae, or other document, demonstrating that the chaplain meets the experience requirements. 3) Copies of transcripts or diplomas documenting that the chaplain meets the education requirements. 4) Copies of certificates documenting completion of Clinical Pastoral Education requirements. 5) If the chaplain seeks to meet the education and/or Clinical Pastoral Education qualification requirements under the equivalencies explained in, the chaplain must furnish a detailed explanation of his/her applicable education and/or training. The North American Industry Classification Systems (NAICS) is 813110 with a size standard: $7.5 million. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 813110. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include: company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, DUNS number, address, point of contact, phone number and socioeconomic category: (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business or Large Business). Additionally: If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractors must be registered in System for Award Management (SAM), formerly CCR (see internet site: http://www.sam.gov).
