Description

The Louisiana National Guard plans to establish a Blanket Purchase Agreement (BPA) with Onsite Dental Care, 1730 S. Amphlett Blvd 123, San Mateo, CA 94402. The contractor shall provide all management, labor, material, equipment, certifications and su pplies necessary to provide all listed services in a HIPAA, Privacy Act, OSHA & ADA compliant mobile facility. Services shall include the following: Periodic Health Assessment (PHA)/Physical Examinations (Over-40, Chapter 2, Flight, etc.): In accordance wi th the AR 40-501 for an undetermined number of soldiers and undetermined armories throughout Louisiana. The contractor must conduct physicals in an HIPAA, Privacy Act OSHA & ADA Compliant Mobile Health Vehicle to include visual acuity, ANCI certified and DOEHRS compatible eudiometry, on-site laboratory testing for lipids, glucose, urinalysis, hcg, and hemoglobins, a complete physical exam by certified clinician, tonometry, and vital signs. All professionals must be licensed and adhere to all requirements of OSHA, State & Federal Regulations is mandatory. (1) OSHA-Compliant Mobile Health Practice (a) 2-Fully Equipped Private Exam Room for full PHA & Physical Exam Requirements (b) Health Stations to include vital signs, Visual acuity, tonometry, DRE (Dig ital Rectal Exam), Hemocult, immunizations, PPD screenings, HIV Blood-Draws, DNA collection, DOEHRs-Compliant audiometry, and all other IMR/FMR requirements (c) DOEHRs Compatible Audiometer (d) TITMUS Visual Acuity Screener(e) CLEA-Waived Rapid L aboratory Screenings (Lipid Profile, Hemoglobin, Blood Glucose, Total Cholesterol, Urinalysis, & Urine HCG) (f) Blood Pressure, Pulse Oximetry, Height/Weight/Temperature (g) Tonometry (h) EKG & Pulmonary Function Testing (i) Optometry with Visua l Refraction (j) Ability to Perform Blood Collection for HIV, DNA, G6PD, RPR (k) Ability to Perform Immunizations & PPD Screenings (l) Staffing = All Necessary Clinicians and medical technicians to Complete Full PHA &/or Physical Exam Requirem ents (2) Portable Medical Equipment (a) DOEHRS Compatible Audiometer Testing: The contractor shall conduct audiometry in accordance with AR 40-501 for an undetermined number of soldiers and undetermined locations throughout Louisiana. Audiometry must be DOEHRS compatible. Adherence to all requirements of OSHA, State & Federal Regulations is mandatory. SPRINT testing will be performed IAW all applicable OSHA, State, and Federal guidelines. (b) TITMUS Visual Acuity Screener: The contractor shall conduct visual acuity testing in accordance with AR 40-501 for an undetermined number of soldiers and undetermined armories throughout Louisiana. Adherence to OSHA, State & Federal Regulations is mandatory. (c) CLEA-Waived Rapid Laboratory Screenings (Lipid Pr ofile, Hemoglobin, Blood Glucose, Total Cholesterol, Urinalysis, & Urine HCG): Phlebotomy/Lab: the contract shall conduct all phlebotomy services such as HIV and G6PD blood draws, pregnancy testing, and pap-smear testing in accordance with AR 40-501 for a n undetermined number of soldiers in an undetermined number of armories & training sites throughout Louisiana. All specimens will be turned over to the Laboratory that is currently under contract with the LAARNG or NGB for testing. The contractor shall u tilize the current accepted shipping method. (d) Blood Pressure, Pulse Oximetry, Height/Weight/Temperature (e) Tonometry (f) EKG & Pulmonary Function Testing (g) Optometry with Visual Refraction (h) Ability to Perform Blood Collection for H IV, DNA, G6PD, RPR: DNA Collection: The contractor shall conduct DNA collection in accordance with the AR 40-501 for an undetermined number of soldiers in an undetermined number of armories throughout the state of Louisiana. The contract will utilize the LAARNG Medical Detachment for all DNA supplies, shipping supplies, and shipping guidance. Adherence to all requirements of OSHA, State & Federal Regulations i s mandatory. (i) Ability to Perform Immunizations & PPD Screenings: Immunizations: The contractor shall conduct immunizations in accordance with AR40-501 for an undetermined number of soldiers at undetermined locations throughout the State of Louisian a. Immunizations shall include, but not limited to, flu, yellow fever, typhoid, tetanus, hepatitis A, hepatitis B, and polio. Adherence to all requirements of the OSHA and all State & Federal Regulations is mandatory. (3) Medical Staffing: (a) Medica l Physician(b) Physician Assistant (c) Nurse Practitioner (d) Phlebotomist (e) EMT (f) Certified Nursing Assistant (g) Licensed Practical Nurse (h) Registered Nurse (i) Behavioral Health Specialist (j) Audiology Technician(k) Optometr ist (l) Ophthalmologist (m) Optometry Technician (4) Medical Supplies (a) Able to Customized Medical Supplies for PHA and/or Full Physical Exams as listed necessary medical services above (b) OSHA Required PPE (c) All Immunizations Serums if necessary (5) Additional Medical Resources: (a) MEDPROS Data Entry. This notice is not a request for competitive proposals, a solicitation or request for offers. Information received as a result of this notice will be considered solely for the pu rpose of establishing additional BPAs for the above services.
