Amendment 0001 for W9128FQ0104 Amendment 0001 The purpose of this amendment is to change the county name from Lyman to Buffalo in the following sections of the solicitation: 1.CLIN 0001 extended description. 2.Place of Performance in the Statement of Work. 3.FAR 52.222-23(e). All other terms and conditions remain unchanged. Updated 7 September 2017 ------------------------------------------------------------------------------------------------- This solicitation is RESTRICTED TO SMALL BUSINESS PARTICIPATION. This project consists of furnishing all personnel, services, equipment, materials, facilities, and all other requirements necessary for milling approximately 3,000 sq. yards of existing asphalt 4 inches deep and replacing it with one 3 inch layer approximately 485 tons of hot-mix asphalt as specified in the performance of work. The new asphalt shall be graded to drain according to drainage plan. Proper care must be taken so that placed asphalt is flush with existing concrete gutter. Catch basin inlets shall be installed in 5 locations specified in the drawings. All work shall be performed in accordance with all pertinent Corps, federal, state, and local regulations. The estimated construction magnitude for this project is estimated to be between $100,000 and $250,000. An organized site visit has been scheduled for 7 September 2017 at 1100. Contractors interested in seeing the site need to make reservation with Michael by telephone (605) 245-1815 or by e-mail at michael.w.werdel@usace.army.mil. All personnel attending the site visit shall meet at the site entrance gate. Offerors: Please be advised of on-line registration requirement for Representations and Certifications Application (ORCA) is now located in System for Award Management (SAM) database https://www.sam.gov/portal/public/SAM/. Representations and certifications are required to be updated annually as a minimum to keep information current, accurate and complete. The electronic annual representations and certifications are effective until one year from date of submission or update to SAM.
