Attachment 4 Quote Submittal Worksheet Attachment 3 Wage Determination, Power Equipment Operator, Pontotoc County Attachment 2 Plan Attachment 1 Drill Hole Locations & Sampling Methods Combined synopsis/solicitation This is a COMBINED SYNOPSIS/SOLICITATION for commercial services prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The project title is Drilling & Field Services in support of a Geologic Investigation. The Solicitation number is AG-SPEC-S-17-0054 and is being issued as a Request for Quotes (RPQ). The acquisition procedures at FAR Part 13 are being utilized. The Government anticipates this solicitation will result in the award of a Firm Fixed Price Contract. Responses are due by 2:00 p.m. Eastern Time, September 15, 2017, to Tami Lanier at tami.lanier@wdc.usda.gov. This acquisition is 100% set-aside for Small Businesses. Location: Upper Clear Boggy Creek Site 26 Flood Water Retarding Structure located in Pontotoc County, Oklahoma. Period of Performance: 30 calendar days. Description of Services. Provide drilling and field services to support a Geologic Investigation (GI) on the high hazard class structure. Refer to the provided Statement of Work for a detailed description of the services and tasks to be provided. FAR Provision 52.212-1 - Instructions to Offerors - Commercial Items (JAN 2017), applies to this project and is incorporated by reference. The following addenda also apply. Offerors must read and follow these additional instructions to receive consideration: a. System for Award Management (SAM) Registration All Contractors must be registered in the System for Award Management (SAM) prior to award of a Federal Contract. However, due to the need for prompt award after receipt of quotes, registration in SAM, to include completion of the Representations and Certifications, and having a status of "Active" is required BEFORE submitting a quote. For information regarding registration visit https://www.sam.gov/portal/public/SAM/. b. Offerors should submit one completed and signed copy of the Quote Submittal Worksheet (Attachment 4) which was developed to facilitate preparation of quotes. i. Section A - Complete by entering company name, contact information and DUNS number. ii. Section B - Prices -- enter total project price. c. Submit responses to non-price Evaluation Factors 1 through 4. d. Sign and date Quote Submittal Worksheet
