This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes only the solicitation; quotes are being requested and a written solicitation will not be issued. The Contracting Officer plans to award without discussions. The Solicitation RFQ VA248-17-Q-1541 is being issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-94 and 2005-95 effective 01-19-2017 rev2. This requirement is set-aside 100% for Service Disabled Veteran Owned Small Business (SDVOSB). The NAICS code for this requirement is 337127 with a small business size standard of 500 Employees. Description of requirements for the items to be acquired: See attachment A Performance Statement of Work (PSW). The following line items pertain to the description as stated: See attachment B - Price/Cost Schedule. Performance Period: From ten (10) days after award date until thirty (30) days after the initial ten (10) days. All services are to be delivered to the Bruce W. Carter Veterans Hospital, 1201 16th Street, Miami, FL 33125. As a part of delivery and acceptance the FOB point is destination. See attachment C - Delivery Schedule. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and offers must fill out price/cost schedule attached. See Attachment D Clauses and Provisions The provision at 52.212-2, Evaluation -- Commercial Items, applies to this acquisition. To be eligible for award offerors must be registered in System for Award Management (SAM) in accordance with the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and additional addenda to the clause are included in Applicable Clauses, Provisions and Addendum. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition; included in Applicable Clauses, Provisions and Addendum. The following additional contract requirement(s) or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices are as follows: 52.252-2 Clauses Incorporated by Reference (FEB 1998) http://www.acquisition.gov/far/index.html (FAR) and http://www.va.gov/oal/library/vaar/index.asp (VAAR) All quoters shall be registered in the System for Award Management (SAM) at https://www.sam.gov; and www.vetbiz.gov, to be considered for an award of a federal contract.
