Description

This is a Pre-Solicitation Notice The Government contemplates releasing the Solicitation in September, 2017. Based on Market Research and responses to the Sources Sought, this acquisition is set-aside for VOSBs which includes SDVOSBs. VA Northern California Health Care System Courier Services Statement of Work General. VA Northern California Health Care System (VA NCHCS) requires the services of a Contractor to provide courier services of laboratory specimens from the Quest Laboratories blood drawing site. Base plus 4 option years Description of Supplies/Services Quantity Price Total Amount Specimen pick up from Yuba City(VA OPC and Quest) to VA Sacramento Medical Center Laboratory (Friday evenings and evenings and prior to weekday holidays) 58 $ $ VA McClellan OPC to VA Sacramento Medical Center Laboratory (Friday evenings and evenings and prior to weekday holidays) 58 $ $ VA Martinez OPC to VA Sacramento Medical Center Laboratory (Friday evenings, prior to weekday holidays and on-call Sundays and estimated 4 emergency transport during a week day) 68 $ $ Total $ 2.0 Period of Performance shall be from October 1, 2017 through September 30, 2018. Option Year 1: October 1, 2018 through September 30, 2019 Option Year 2: October 1, 2019 through September 30, 2020 Option Year 3: October 1, 2020 through September 30, 2021 Option Year 4: October 1, 2021 through September 30, 2022 3.0 Place of Performance and Hours of Operation Contractor shall pick up laboratory specimens from the Quest Laboratories blood drawing site located at 866 Plumas St., Suite B, Yuba City, California between 4:15pm and 4:30pm on Fridays (and the evening prior to federal holidays if the holiday falls on a weekday) and deliver the specimens to the Mather Laboratory, Building 650, 10535 Hospital Way, Mather, California Friday evening by 5:30 PM (call ahead to laboratory 916-366-5373 if traffic or vehicle issues will delay delivery). Contractor shall pick up laboratory specimens from the VA McClellan Outpatient Clinic Laboratory located at 5342 Dudley Blvd, Building 98, McClellan Air Park, California between 4:15pm and 4:30pm on Fridays (and the evening prior to federal holidays if the holiday falls on a weekday) and deliver the specimens to the Mather Laboratory, Building 650, 10535 Hospital Way, Mather, California Friday evening by 5:30 PM (call ahead to laboratory 916-366-5373 if traffic or vehicle issues will delay delivery). Contractor shall pick up laboratory specimens from the Martinez Outpatient Clinic Laboratory located at 150 Muir Road, Building 19, room B233, Martinez, California between 9:30am and 10:00am on Saturdays and deliver the specimens to the Mather Laboratory, Building 650, 10535 Hospital Way, Mather, California the same day, no later than 12:00PM (call ahead to laboratory 916-366-5373 if traffic or vehicle issues will delay delivery). In the event that there are specimens to be picked up on Sunday, the pick-up site will notify the Contractor in advance that delivery service is required for that day and on-call for evenings prior to a holiday or weekday emergency pick up to the Mather Laboratory, Building 650, 10535 Hospital Way, Mather, California the same day within 4 hours. In the event that there are no specimens to be picked up, the pick-up site will notify the Contractor in advance that delivery service is not required for that day. When advance notification is given, VA NCHCS will not be charged for the delivery service. 4.0 Points of Contact: 4.1 COR Contracting Officer s Representative Martha Goodwin at (916) 843-7235 or email Martha.goodwin@va.gov. 4.2 CO Contracting Officer 5.0 CO Responsibilities: 5.1 The CO is the only person authorized to approve changes or modify any of the requirements of this contract. The Contractor shall communicate with the CO on all matters pertaining to contract administration. Only the CO is authorized to make commitments or issue any modification to include (but not limited to) terms affecting price, quantity or quality of performance of this contract. 5.2 The CO shall resolve complaints concerning Contractor relations with the Government employees or patients. The CO is final authority on validating complaints. In the event the Contractor effects any such change at the direction of any person other than the CO without authority, no adjustment shall be made in the contract price to cover an increase in costs incurred as a result thereof. 5.3 In the event that contracted services do not meet quality expectations the best remedy will be implemented, including but not limited to, a targeted and time limited performance improvement plan; increased monitoring of the contracted services; consultation or training for Contractor personnel to be provided by the VA; replacement of the contract personnel and/or renegotiation of the contract terms or termination of the contract. 6.0 COR Duties and Responsibilities: 6.1 The COR will be appointed by the CO and a copy of the appointment letter outlining the responsibilities of the COR will be provided to the Contractor. 6.2 The COR shall be the VA official responsible for verifying the contract compliance. After contract award, any incidents of Contractor noncompliance, as evidenced by the monitoring procedures, shall be forwarded immediately to the CO. 6.2 The COR will be responsible for monitoring the Contractor s performance to ensure all specifications and requirements are fulfilled. 6.3 The COR will review and certify monthly invoices for payment. If, in the event the Contractor fails to provide the services in this contract, payments will be adjusted to compensate the Government for the difference. 6.4. All contract administration functions will be retained by the VA. 7.0 Contractor Qualifications: 7.1 Contractor must be a courier company with a state license and carry liability insurance. All couriers transporting specimens under this contract must be bonded and carry cellular phones and pagers for communication purposes. Any delays in the delivery of specimens are to be reported to the Mather Laboratory at (916) 843-5373. 7.2 Contractor s couriers transporting lab specimens must be trained in Universal Precautions in the handling of biohazard materials (specimens). Training must include the use of procedures in case of biohazard spills and accidents. 7.3 Contractor s couriers transporting lab specimens must be trained in Universal Precautions in the handling of biohazard materials (specimens). Training must include the use of procedures in case of biohazard spills and accidents. 7.4 Contractor personnel must be able to provide proof of successful completion of training and certification as requested by the COR. 8.0 Contractor Responsibilities: 8.1 Contractor shall provide courier services of laboratory specimens from the Quest Laboratories blood drawing site. The contractor shall provide the following services: 8.1.1 Laboratory specimens, i.e. body fluids (blood, urine and stool), will be prepackaged in biohazard bags with absorbent material. 8.1.2 Courier will provide containers to ensure specimens remain at correct temperatures (room temperature, refrigerated, or frozen) during transport 8.1.3 All specimens must be transported in accordance with all Department of Transportation and OSHA Codes of Federal Regulations (CFR) 49 and 29. 9.0 Identification, Parking, Smoking and VA Regulations 9.1 Contractor personnel performing work onsite at VANCHCS facilities shall wear visible identification at all times while on the premises of VANCHCS, including wearing a temporary visitors badge, which will be issued by VA police upon Contractor s arrival at VA NCHCS. All personnel entering the VA facility may have to consent to inspection of all categories of packages as a condition of admittance. 9.2 It is the responsibility of contractor personnel to park in the appropriate designated parking areas. Information on parking is available from VA Police. VA will not invalidate or make reimbursement for parking violations under any conditions. 9.3 Smoking is prohibited in any VANCHCS building and the surrounding grounds, including the parking lots. Smoking is permitted only in a designated shelter. 9.4 Intoxication, debilitation resulting from drug use, insubordination, theft, patient abuse, dereliction or negligence in performing directed tasks and possession of weapons is prohibited and grounds for immediate removal from VANCHCS by the VA Police. All personnel entering the facility may have to consent to inspection of all categories of packages as a condition of admittance. 9.5 Violations of VA Regulations may result in a citation answerable in the U.S. Federal District Court. 10.0 RELEASE OF MEDICAL INFORMATION. 10.1 The Contractor shall not release any medical information obtained during the course of this contract to anyone outside the VA, unless authorized in writing by the VA COR and the patient, as required by applicable law. 10.2 The Contractor shall ensure the confidentiality of all patient information being transported and will be held liable in the event of breach of confidentiality. The Contractor shall comply with the provisions of the Federal Privacy Act of 1974 (Public Law 93-579). 11.0 PROFESSIONAL LIABILITY INSURANCE. 11.1 Professional liability insurance shall be by a commercial insurance company in the business of providing the required insurance coverage. The Contractor shall provide a copy of the Insurance Certificate upon receipt of a written request. 12.0 SAFETY. 12.1 The Contractor shall comply with all applicable OSHA, Federal, State, laws, and regulations as required for performing the type of services required. 12.2. The C&A requirements do not apply, and that a Security Accreditation Package is not required.
