The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Health Care service purchase order for a Physician Assistant or Family Nurse Practitioner to provide services to patients with a diagnosis of pre-diabetes at the Rapid City PHS, IHS Hospital, Rapid City, South Dakota. This is an option year service contract; the base year period of performance will be from date of award through September 30, 2008 or 200 hours; whichever comes first. The option year will be from October 1, 2008 through September 30, 2009. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of this purchase order. Request for RFQ package shall be in writing either by email or fax and it will be made available approximately fifteen (15) days after the date of this publication which will be January 18, 2008. The closing date of this RFQ package is February 4, 2008.
