Description

This is a request for quote (RFQ) for all services to perform the duties of a Registered Nurse for the Health Unit located in the Ron Mazolli Federal Building, Louisville, Kentucky. Services will be from 01 February 2008 thru 31 January 2009. This RFQ contains an option clause to extend the contract for four one-year periods. If the options to extend are exercised, the four one-year periods will be from 01 February 2009 thru 31 January 2010, 01 February 2010 thru 31 January 2011, 01 February 2011 t hru 31 January 2012 and 01 February 2012 thru 31 January 2013. NOTE: This procurement is 100% set aside for small business, NAICS 541990 ($6.5 million). Award for this procurement will be based Best Value to the Government. Factors to be considered are Experience, Past Performance, and Price. This RFQ has an approximate issue date of 10 December 2007 and a due date of 10 January 2008 at 2:00 PM (local time).
